2010 ASA Roster form / Waiver & Release form

I thought I had an extra copy of this form but I dont. My team plays in a tournament this weekend and I need it. Does anyone have this or know how to find it online? I went to ASA.COM and had no luck. If anyone has it and could email it to me I would greatly appreciate it! shanekirschner@msn.com

Someone please help me with this if possible! I would like to think the tournament would have some copies of this, however I cant count on that.

It is hard to get anything from that website. I've been trying to reach someone from national office in Oklahoma and no one will return my call on an eligibilty issue.
