Team Code fo Conduct

Just wondering how many teams/orgs made parents/players sign a "code of conduct". If your team/org uses one, any chance I could get a copy? We are instituting one and I would much prefer to have an existing document to work off of than start from scratch.

We did. ?I believe that I have a copy at home that I could scan and send. ?We also signed one for my other DD's travel s-occer team. ?I know that I have that one.
