Evaluation Forms

Does anyone have a evaluation form they could send me? I am looking for one that has muliple players on a sheet and related to softball that includes infield/outfield - quickness ball transfer etc. I found one I like but I would need to retype it to get it to be legible. Any help would be appreciated.

What is your purpose? Are you asking for a scouting form or a tryout form? I'm guessing the latter as HS season tryouts are coming up.

A tryout form. I am coaching our middle school team this year. I used to have all this stuff and threw it all out thinking I wouldn't need it. Thats the way it always is...
