Could be to heavy of a bat. An endloaded bat. A casting out swing.
Poor timing.

Sounds like she isnt keeping her weight back on her back leg? Get some Tee work in and some one on one coaching to point out each time what it looks like she is doing. If she has multiple issues fix one first then move to another. Dont try and fix them all at once or it will likely be too much for her. Hands (butt of bat) to ball first then roll wrists. Really focus on her needs while practicing after all thats why we practice.

There is a whole bag of tricks that you can try and depending on the moment in time at the tee you pull one or the other out.

One of my favorites is to use a broomstick to add resistance to the front hip so the batter can feel that they are going too far forward. Stand behind the batter and extend the stick across the hip and let her take some cuts on the tee. You feel the pressure and so will she. Film it if you can so she sees it. Not sure you use a hip shift but if you do account for that in a little forward hip (but not upper body) movement.

Another one I use is to get her in her stance and have her look straight down. Place a rock or another marker on the ground. On the tee have her freeze when she completes her swing and look down again. Is she still over the rock?

Those are two things you can do for HER to feel/see what is going on.
