Penalties come under By-law 12 (http://www.ohsaa.org/general/about/Bylaws.pdf) unless otherwise specified.

BYLAW 12 ? PENALTIES
12-1-1 Penalties for violation of the OHSAA Constitution, Bylaws and Regulations shall be imposed by the Commissioner or another administrative staff member designated by the Commissioner.

12-1-2 Penalties include: suspension, forfeiture of games, forfeiture of championship rights, probation, public censure, denial of participation or fines not to exceed $2,500 per occurrence or such other penalties as the Commissioner deems appropriate.

In the softball specific rules (http://www.ohsaa.org/sports/rglts/SB06-07.pdf), however, it is stated:

7) Penalties
7.1) The maximum penalty for violation of Bylaw 10-3-1, participation with a noninterscholastic fast pitch softball squad while a member of the school?s interscholastic fast pitch softball squad, is ineligibility for the remainder of the interscholastic fast pitch softball season.

7.2) The maximum penalty for violation of any OHSAA regulations for any member of a school squad except a senior, shall be ineligibility for fast pitch softball the ensuing season. If a senior, the maximum penalty shall be ineligibility for the remainder of the school year. (See Bylaw 10-4-1 for penalty for playing on a noninterscholastic team.)

7.3) The penalty for a coach for violation of Bylaw 10 (Non-Interscholastic Teams) may include suspension, probation, public censure, fine not to exceed $1,000 per occurrence, or such other penalty as the Commissioner may deem appropriate.

Thus, at least the way I read this, 7.2 would apply to the girl and the penalty could be loss of eligibility for potentially the entire next season (unless you want to argue that, as a 6th grader, she's not a member of the squad and throw yourself at the mercy of By-law 12). Penalties for the school and coaches are a little more open ended since rule 7 doesn't apply in this situation, and jurisdiction would fall back to By-law 12, but could conceivably include forfeiture of games played that season, and/or probation and fines.
