Quick question on concussion test .

I can not seem to get an answer , do you have to take the concussion test over every year ? Is the test just a one time thing and good forever ?

As edge said, it is good for 3 years, and can be used for multiple sports. I use the same certificate for Softball, Baseball, and Football.
